The cab cards are still new to some of you. Now the SHV will be on the cab cards (If the states
printers work). If you need SHV stickers, these will now be on the cab cards, no more half page
sticker. Be careful to order or cancel for each unit as needed. If you need to replace a SHV
sticker you will need to pay for a replacement cab card.

Then you will need to: Sign application, provide total miles for each state. Verify all states you
need have the correct weight. Verify all unit numbers and vehicles are correct. Attach 2290
(h.v.u.t) in correct name for each truck. Verify phone number and address (if different, attach new
proofs). Show proof of corporation in good standing if you are an incorporation. Make sure
U.S.DOT number is on application and in correct name. Everything must match.

2)Utility bill,
3) Copy of real estate tax bill or rental contract. If you are living with family, write a letter
explaining arrangements along with a copy in their name.
4) Insurance statement, some legal form, etc.

If incorporated need proof of good standing
ICC authority number
Proof of ownership ( title)
Proof of insurance
Federal employers ID number or social security # (FEIN)
A list of the states you want to run in and why.
Weight you want to run at.
Unit numbers for each truck.
Copy of lease if you are leasing vehicles.
